Responsible for assessment of health needs and contributing to the plan for care for individuals and families. ElderONEAn affiliate of Rochester Regional Health, a Program of All-Inclusive Care for the Elderly (PACE). Servicing Monroe, Ontario and Wayne counties, ElderONE is an innovative alternative to long-term nursing home placement. As the area's only PACE program, ElderONE represents the future of senior health care. This long-term care program provides the entire continuum of medical care and support services to seniors 55 and older with chronic care needs, allowing them to maintain their independence in the comfort of their home without compromising their health or safety.RESPONSIBILITIESPatient Care. Conduct initial assessments; establish plan of care and provide prescribed treatments and demonstrated nursing abilities; coordinate the delivery of 24-hour care;Communication. Counsel and guide individuals and families towards self-help in recognition and solution of physical, emotional and environmental problemsEvaluation & Documentation. It may be modified in the future and eligible for additional pay components.
